Special Programs

Is South Florida your second home?  Do you travel often to South Florida?  Do you need medication refills?  Do you need to check your labwork?

• Patients who have planned extended stays in South Florida who will need medication refills, laboratory monitoring and other primary care needs during their stay in South Florida.

1. Call or email our Patient Care Specialists to ensure availability in the Vacation Doc Program.
2. Based on the availability of the Vacation Doc Program, we will register you to our concierge service for the duration of time specified by your physician.
3. Once enrolled, we will communicate with your primary care physician and specialists for copies of your updated office visits, lab results and for further recommendations.
4. Once completed with your care in South Florida, we will provide you wi a physical copy of your laboratory and imaging reports, our consultations, specialist consults and reports of procedures for your own records and surgical reports.

Price of the 'Vacation' Program is $250/month.

This is a special collaboration between Intrepid Family Medicine and physicians abroad to help their patients while visiting South Florida.

• Patients who have planned extended stays in South Florida who will need medication refills, laboratory monitoring and other primary care needs.
• Patients that require testing, imaging and/or medications not available in their home country.
• Patients that require referrals and help seeing specialists not available in their home country.

1. Have your primary doctor or specialist email us at intrepidfamilymedicine@gmail.com with a brief synopsis of your current medical problems and needs and during which time frame these services would be needed..  
2. Our Patient Care specialists, will work in conjunction with your physician to ensure that we can provide the services needed.
3. Based on the availability of the Mon Doc Program, we will register you to our concierge service for the duration of time specified by your physician.
4. Once enrolled, we will keep in close communication with your physician with updates on results and for further recommendations.
5. Once completed with your care in South Florida, we will provide you wi a physical copy of your laboratory and imaging reports, our consultations, specialist consults and reports of procedures for your own records and surgical reports.

Price of the 'Mon Doc' Program is $250/month.

This program was built for the uninsured patients that are currently admitted to a local South Florida that need to be followed short term until they establish a relationship an outpatient Primary Care Provider.

• Discharge uninsured patient's safely with peace of mind knowing that they will be closely followed after discharge.

• Before you discharge your patient:

1. Our Patient Care Specialist will talk to your case manager to ensure that we will be able to provide the care that the patient requires in the outpatient setting (testing, imaging, referrals, etc...
2. Once the patient is agreeable to the Post-Acute Bridge Program, completes registration and our Patient Care Specialist confirms availability in the program. our physician will be available for sign-out.
3. Our Patient Care Specialists will work in conjunction with Case management to ensure that we receive all discharge paperwork.  We will provide your patient with a physical and digital copy of his/her discharge paperwork, Office notes while in program to the physician or clinic taking over the care of the patient once they have established care.

• Decrease readmissions by having a colleagues in the community who will ensure a smooth transition of your patients.  Imagine no more readmissions because:

               -The patient's did not receive their script

               -The patient's nebulizer machine was not delivered.

               -The patient could not get an appointment in time...

Price of the Post-Acute Bridge Program is $250/month.

• Uninsured and need to be followed up outpatient but you do not currently have insurance, you do not have a primary, you can't get in to see your primary in time for follow up.   You can now leave the hospital knowing that your care teams recommendations will be closely followed after discharge until you establish care with your Primary Care Doctor.

• Benefits to you:  Our physician will talk to your In-Patient care doctor for a smoother transition. 1 Hour post hospitalization visit to review your discharge paperwork, consults, lab and imaging  results on day recommended on discharge paperwork.   Establish a detailed Post Acute Care Bridge Roadmap with guidance of discharge paperwork.  Monitor lab work and vitals after discharge.  Unlimited follow up visits as needed for 30 days.  Once you establish  care with your Primary Care Provider, All your paper work will be transferred over.

             This program is only for 30 days.  This provides you with the opportunity to establish care with a Primary Care Provider.  If there are situations beyond your control, and you need to extend your bridge program
